1. Open up a new canvas 100x100 px. Fill it with black.

2. Paste your cropped, resized picture on top of the black layer (do not merge the layers).

3. Erase around the edges of your picture using a soft eraser (doesn't have to be perfect, just as long as the edges are gone)

4. Create a new color balance layer, and enter the following:
Midtones: +78, 0, +28
Shadows: -50, +23, -18
Highlights: -21, -39, +33

5. Create a new color fill layer, #390c0c, and set it to exclusion.

6. Create new selective color layer, and enter the following:
Reds: -37, 0, +100, 0
Yellows: -36, +22, -11, 0
Whites: 0, 0, -50, 0

7. Add text. I used the font Cairo and a white color, and I set this layer to Soft Light. After that, I rotated it.

Other Examples:

*For the last two icons (nathan and haley) I had to screen my base a few times to get them =)
